From April 1, 2026, NHS England is introducing significant changes to treatment plans, claims, and UDAs. Key updates include replacing Urgent Treatment with Unscheduled Care, restructuring Denture treatments, moving Fissure Sealants from Band 1 to Band 2, introducing Nurse-Applied Fluoride Varnish, and updating patient charges across all bands. CareStack will automatically reflect these changes as of April 1, 2026.
Unscheduled Care replaces Urgent Treatment from April 1, 2026, operating as a standalone treatment plan with a Band 1 patient charge of £27.90 and an NHS payment worth £60 in UDAs (based on the practice's notional UDA value). An additional £15 fixed fee is paid monthly if the contract requires emergency prioritisation under the Required Number of Urgent Treatments service line on Compass. Treatment plans must not be opened before April 1, 2026, as earlier claims will be rejected.
Denture Repair now earns 2 UDAs (twice the previous value) with a zero patient charge and must be mapped as Unbanded. The retired Denture Additions/Reline/Rebase category has been replaced by two new Band 2 treatments (Denture Additions and Denture Reline/Rebase), each earning 3 UDAs standalone. When part of a Band 2 course of treatment, practices earn an extra 2 UDAs on top of standard Band 2 UDAs, totalling 5, 7, or 9 UDAs depending on the sub-band. When concurrent with a Complex Care Pathway, the combined course pays CCP UDAs plus 2 UDAs.
Yes. From April 1, 2026, qualified Dental Nurses can deliver Nurse-Applied Fluoride Varnish as Direct Access DCPs for patients under 16, earning 0.5 UDA with zero patient charge. The Dental Nurse must hold a valid Personal ID number, be registered on the practice's Compass contract, and the claim must contain only the Fluoride Varnish treatment.
Complex Care Pathways are structured, long-term treatment plans for high-needs patients that require monthly claim submissions with strict sequencing rules. Three pathways are being implemented in 2026: Pathway 1: ≥5 teeth with caries into dentine; Pathway 2: Pathway 1 plus existing unstable periodontal disease; Pathway 3: Pathway 1 plus a new unstable periodontal disease.
Each pathway requires multiple sequential submissions: a sign-up, monthly interim declarations, and a final submission (complete, suspended, or incomplete). Missing or out-of-sequence submissions will trigger rejections and paused payments.
Each pathway has a fixed total value, paid monthly: Pathway 1: £284 across 6 submissions; Pathway 2: £709 across 12 submissions; Pathway 3: £248 across 6 submissions.
Most routine treatments must be delivered within the pathway itself. The only concurrent treatments allowed are Denture Repairs and Modifications (Reline/Rebase/Additions), Band 3 Treatments, and Unscheduled Care (to be confirmed).
